Here are 3 samples of our new "Angel Wing Plaques", (Pink, Blue,or White). You could use White as gender unknown if you like. To have your memorial site linked to your Angel's Plaque, make sure you fill out the form with the correct website address and we will be sure to include it for you.

Here you can request Angel Wings for your Angel or Angels, to be displayed on our website. We will email you a copy also. Please fill out the form below and be sure to include all the correct information as you want to see it on the Memorial Plaque. The name/names will appear exactly as you fill it out on the form (double check for typo's) Also if you do not pick a color, then white will be assigned to the Memorial Plaque. If you put your last name but not your Angels, (even if it's the same) then we will not put a last name on the plaque either. Please fill out 1 request for each Angel, unless they are twins and you want their names on the same Memorial Plaque. We are honored to light a candle and display their Memorial Plaque.
